Sports Preparticipation Physicals

     If your child is in or about to enter the Middle School and is interested in trying out for any of the many sports offered during the school year, we are encouraging you to consider a physical examination by your own physician in keeping with the state's recommendations.  Physicals may be done 365 days prior to the first practice session.  We would encourage you, to have a physical done during summer vacation, or take this form to your child’s well visit.  The required sports physical form is available on the Upper Township Middle School web page at http://www.upperschools.org.  Go to the Middle School, then to the nurse office, Ms. Sackett’s web page, click on Forms, then click Sports Preparticipation Physical Form.  Forms are also available in the main office.  You can also get the forms by emailing me at sackett@upperschools.org.  I do check my email throughout the summer.

 

     To assist you in the decision making process, we are providing a brief overview of the time frame for our offered sports;

 

      Sept- Field Hockey, Soccer and Cross Country (Forms are due by Wednesday, September 14)

      Nov. - Girls and Boys Basketball

      Jan. - Volleyball for eighth grade students

      March - Baseball, Softball, Track and Golf.

 

     If you are unable to provide a "home based physical" prior to the sport, we will continue to offer school-based sports physicals at the appropriate times during the school year, as indicated in the guidelines.  Please contact Ms. Sackett to make arrangements before the date physicals are due.  All physicals must be done on the approved school form and all forms must be complete.  If you have any questions, please contact the middle school nurse at 628-3500 ext. 2247 or email sackett@upperschools.org. Thank you for your cooperation.

Vaccination Requirements for Students Entering Sixth Grade or transferring from another state/country
All pupils entering Grade 6 on or after 9/1/08 and born on or after 1/1/97 are required to have a Tdap booster and a Meningococcal vaccination.  Please forward any vaccinations to Ms. Johnson, if given before June 23rd, or to the Middle School Health Office.  Any questions, please call or email me.

PANDAS Information and Link
PANDAS, is an abbreviation for Pediatric Autoimmune Neuropsychiatric Disorders Associated with Streptococcal Infections. The term is used to describe a subset of children who have Obsessive Compulsive Disorder (OCD) and/or tic disorders such as Tourette Syndrome, and in whom symptoms worsen following strep infections such as "Strep throat" and Scarlet Fever.

The children usually have dramatic, "overnight" onset of symptoms, including motor or vocal tics, obsessions, and/or compulsions. In addition to these symptoms, children may also become moody, irritable or show concerns about separating from parents or loved ones. This abrupt onset is generally preceded by a Strep throat infection.

Illnesses and Absence
If you child is ill and will be absent from school, please call 628-3500 and follow the prompts for the attendance line.  (Press 5 for the Middle School, then press 1 for the attendance line.)

It is important that you call each day your child will be absent.  If your child is sent home from school with a fever, you do not need to call the next day.  Any following days out need to be called in to the attendance line.

If your child is absent due to illness for 5 days, a medical note may be required.

If your child has a fever, flu-like symptoms or is vomiting, please call me at extension 2247 and refer to my announcement on What to do about the flu.

Students may not return to school until they have been without fever for 24 hours without the use of fever reducers such as Tylenol(acetaminaphen) or Motrin(ibuprophen). 

If vomiting, the student should remain home for 24 hours from the last episode.

Frequent hand washing and covering coughs and sneezes help curtail the spread of illness.
